Haemorrhoids are vascular structures in anal canal that can get swollen and inflamed. Internal haemorrhoids are present above the dentate line and presents with painless, bright red rectal bleeding when defecating. External haemorrhoids are present below the dentate line and result in pain and swelling in the anal region with darker bleeding.
In about 40% cases of pathological hemorrhoids there are no significant symptoms. Typical symptoms associated with hemorrhoids are pain, bleeding, pruritus, burning and swelling. On examination, skin tags, fistula or fissure, prolapsed or thrombosed hemorrhoids and blood can be found.
Internal hemorrhoids often result in painless, bright red rectal bleeding during or after defecation. Blood usually covers the stool, called hematochezia, is on the toilet paper or drips into the toilet bowl. Stool is normal coloured. There may be an associated mucous discharge, perianal mass, itchiness and fecal incontinence.
External hemorrhoids, when thrombosed, often result in pain and swelling in the area of the anus which resolves in 2-3 days. It can also cause itching in the surrounding anal region.
Anal fissure is a break or tear in skin of anal canal and presents with bright red anal bleeding. It can be painful when acute. Untreated fissures can develop a skin tag called sentinel piles which covers the fissure and causes pain and discomfort. Fissure extends from the anal opening and is usually located posteriorly in the midline. Avoiding straining when defecating, maintaining proper hygiene, high fibre diet and good intake of water are some measures that can help in prevention and management of anal fissures.
